The Sister Mary Therese Esselman Center for Rehabilitation

We consider Rehabilitation a path we share with our residents, one that combines clinical expertise with compassion for the healing process. The journey begins with each resident and the rehabilitation services that his or her physician agrees will help them regain strength, improve mobility and acquire a feeling of greater stability in his or her daily activities. The Center was named in 2011 to honor Sister Mary Therese, a Carmelite Sister who dedicated 10 years of her religious service to the residents and families of St. Ann’s.

We are committed to this interdisciplinary-team approach to care, keeping each resident and his or her family at the center of the care continuum, as did Sister Mary Therese. The St. Ann’s Rehabilitation team works closely with the onsite interdisciplinary team, to help residents learn more about ways to cope with the life changes that may occur before, during or after rehabilitation. Our commitment is to help our residents return to the best possible physical level and quality of life as soon as possible, and to help each one return home or progress to a more appropriate living space.

St. Ann’s Center for Rehabilitation is well equipped to address the range of challenges associated with aging and includes rehabilitation beds within our campus, on 23 acres of Grand Rapids’ West Side. Our typical resident is with our Rehabilitation program for less than 30 days, and throughout his or her stay we hold regular care conferences to help ensure quality of care and to ensure the resident and team are on track. Services are available seven days a week and on holidays if needed.

St. Ann’s has expert discharge planning services staff, who facilitate home evaluation, referrals to other key community resources, help with scheduling appointments, and post-discharge follow up check-ins. Our Rehabilitation Department is staffed by highly qualified clinicians, including Physical Therapists, Occupational Therapists, and Speech Therapists. The Rehabilitation Team is part of an excellent interdisciplinary team that works together to meet the needs of all residents.

We start rehabilitation at admission and assist each person right through discharge with tools that provides each resident to continue their success even after they are discharged from therapy. Our goal is not only to get each resident back to their prior level of functioning, but to maintain independence once he or she returns to the community.

Physical Therapy will focus on strength, balance, functional mobility skills, range-of-motion, stability, pain management, fall-risk prevention, caregiver education, home assessments, and community re-entry skills.

Occupational Therapy will focus on dressing, bathing, toileting, self-feeding, home management, cooking, shower transfers, pain management, adaptive equipment instruction, compensatory techniques, home evaluations, positioning, and home modifications.

Speech Therapy focuses on speech and language interaction, including articulation, voice quality and voice strength; cognitive retraining including memory, problem solving and judgment; and swallowing skills, including compensatory techniques to reduce the risk of aspiration and diet recommendations.

Our Rehabilitation staff will do a comprehensive home evaluation before residents return home and will provide recommendations to assure continued safety and success at discharge. We involve family and caregivers with any special training that might be needed, and will outline home exercise programs designed to help individuals maintain the strength they gained during rehabilitation. We also include key community resources, to ensure that every person’s compete needs are met at the time he or she is discharged.
